The Scalp-Perimenopause Connection: What to Expect and How to Cope

As a dermatologist, I've seen firsthand how hormonal changes can impact scalp and hair health, particularly during transitional periods like perimenopause. Women in their 40s often begin to notice a myriad of changes in their scalp – increased flaking, sensitivity, oiliness or dryness. If this sounds familiar, you're not alone. Here's what's happening and what you can do about it.
During perimenopause, estrogen and progesterone levels start to fluctuate and eventually decline as the body prepares to transition out of its reproductive years. These hormonal shifts can trigger an imbalance in scalp sebum production, leading to either an oily or dry scalp environment that becomes more prone to flaking and irritation.
At the same time, perimenopausal hormone changes can increase scalp skin sensitivity, so even your trusted haircare products might start to cause itching or redness. Many women also notice textural changes in their actual hair during this time—it may become dryer, coarser, or more brittle.
So what can you do? First, acknowledge that these changes are real and warrant attention. You may simply need to make some adjustments to better support your scalp during this transition:
Use a gentle, balancing shampoo to help regulate sebum production without stripping the scalp of its healthy oils.
